The first 5 days:
If you are off work for up to 1 week (5 working days) you do not need a sick note from your GP. You can download a form here which is called a Statutory Sick Pay Form or Employees Statement of Sickness (form SC2).
After 1 week:
Once you have been absent from work for more than one week you can obtain a sick note from your GP. Please contact the Surgery on 01704 531114 to request a routine telephone appointment, a consultation will need to take place if you have not been seen by a Doctor before for the current problem and it is not a continuing condition.
If the problem persists and you require a further sick note, you must contact the Surgery, a member of the reception team will ask a series of questions which will then be reviewed by a clinician prior to the sick note being authorised or should a telephone consultation be required, the Practice will contact you to arrange a further appointment.
Sick notes can be collected at the Practice or alternatively emailed or sent via SMS text message.
