We want to understand what these changes mean for you and we invite you to complete a short online survey to gain your views. This will help us to understand if there is anything we need to consider, or any measures we need to put in place when we permanently close Roe Lane Surgery.
We also want to know about your experience of using our services throughout the whole of the COVID-19 pandemic. So, we are also asking you to answer some questions about the systems that all practices put in place during this period to keep patients and staff safe.

What happens next
How we will use your feedback
All the views we gain will inform our closedown plan for the Roe Lane Surgery building. They will help us to make any reasonable adjustments to support patients with this change.
It is important that we hear from as many patients as possible. We have carried out an equality impact assessment and we will be talking to those who may be impacted most by the closure of Roe Lane Surgery.
Additionally, your experiences of using our services during the COVID-19 pandemic will also help us to make improvements to the practice into the future.
